Baldur’s Gate 3 have a stone disc puzzle inside the Defiled Temple. Which is in the Shattered Sanctum. Its exact location coordinates are X:436, Y:52.
I am bad at solving problems that are complex… The puzzle itself is quite long and difficult. So, how to solve stone disc puzzle in Defiled Temple?
In order to unlock the moon door, you must make full 4 moons on the top disc.
Once you solve the puzzle, your character says:
Wonder what’s down there. Guess there’s only one way to find out!
The door leads you to the Selunite Outpost. Which is inside the Underdark. This puzzle unlocks one of the secret BG3 Underdark entrances.

I mean, you don’t really need this guide to solve it. Thinking 2-3 moves ahead is all it takes. Trust me it will be much more satisfying. However, if you don’t want to waste time, you can use this guide to cheat. Just this once.
All in all, the worst thing about this puzzle is the disc turning speed. They spin really slow which is quite frustrating. I guess realism in gaming can be boring sometimes.
